河南：中医药抗艾滋病疗效让世界瞩目

编者按：中医药治疗艾滋病看河南。艾滋病病例最多、中医药治疗介入最早、中医药大省赋予的雄厚中医药实力及其临床救治、科研设计的完善，使得河南在中医药治疗艾滋病领域居有着独一无二的地位。河南的经验和成果不仅让众多艾滋病患者看到生命的希望，也让世界为之瞩目。本报记者日前深入河南中医药抗艾一线，走近那群或将为全球抗艾做出巨大贡献的中医专家们。

生意社10月19日讯上世纪90年代，在中原大地的部分农村地区，艾滋病魔悄然而至。众多艾滋病病人生命安全受到危害，人民群众心头阴霾重重。在党和政府的领导下，医疗卫生工作者挺身而出，抗击病魔，护佑百姓。一批又一批中医人走上治艾一线，以中医药为武器与病魔“短兵相接”。特别是在国家中医药管理局开展中医药防治艾滋病试点工作之后，河南省作为首批试点省之一，加强项目管理，组建专家团队，完善诊治方案，健全工作机制，稳步规范实施，使中医药治艾取得确切的临床疗效，5年来，数千名纳入项目管理的患者重启希望之舟。

进入治艾主战场

从以关怀性治疗为主到以医疗救治为主，中医药治疗艾滋病以确切疗效赢得广泛认可，成为全省艾滋病防治工作体系的重要一环

上世纪90年代中后期，河南一些地方艾滋病呈现高发态势，绝大部分患者集中在农村地区。河南省中医管理局曾多次组织专家调研、选方遣药，但由于种种原因，中医药在抗艾中一直以关怀性治疗为主。

直到2003年，中医药抗击“非典”取得世人瞩目的成果，加上一些民间中医积极探索治艾并显示出独到疗效，中医药防治重大疾病和传染病的思路和价值被重新审视。为此，国家中医药管理局派人赴河南作深入调研，2004年8月率先在河南等5个省份启动中医药治疗艾滋病试点项目。

河南省卫生厅副厅长、省中医管理局局长夏祖昌牵头组织河南中医学院三所附院和省中医药研究院抽调多领域骨干专家成立中医药防治艾滋病专家组。

经过培训学习，20来人的专家组一起下到开封市尉氏县作临床治疗，验证摸索。两个多月，通过疗效观察发现，接受中医药治疗的患者症状改善，食欲增加，体质增强，康复信心提高。专家们开始找到对付艾滋病的感觉了。

试点项目主要面向三类患者，一是不适应抗病毒治疗的，二是退出抗病毒治疗的，三是接受抗病毒治疗但毒副反应较大的，为他们提供免费中医药治疗。2004年10月，项目正式实施，以患者自愿加入为原则。国家分配给河南的项目计划数是1400例，而当年实际上有9个县市的1792名患者纳入。到2006年，项目进一步扩大，病人增加至2546例，分布在开封、周口、驻马店、南阳、商丘5个省辖市的13个县（区）、37个乡（镇）、137个行政村。今年，省卫生厅决定，将1000名退出抗病毒治疗的患者转为中医药治疗。

5年来，中医药治疗艾滋病以确切的疗效，赢得了患者的广泛信赖，试点项目已成为河南省艾滋病防治工作体系的重要一环。根据省中医药管理局的阶段性总结，中医药治疗艾滋病使艾滋病人临床症状改善明显，生活质量有显著提高；艾滋病人机会性感染发生频次明显减少；中医药具有稳定和提高艾滋病人CD4阳性T淋巴细胞计数作用；中医药治疗对艾滋病人病毒载量有一定影响。中医药积极参与艾滋病防治工作，为不能接受抗病毒治疗的病人提供了关怀治疗，增加了艾滋病临床治疗手段，在一定程度上扩大了救治范围，降低了治疗费用，同时提高了病人对抗病毒治疗的依从性，为构建符合河南省情的艾滋病防治工作体系起到了积极作用。河南试点项目工作取得了阶段性成果，得到国家中医药管理局的充分肯定。

河南中医学院三附院李发枝教授说，经过中医药治疗的患者，痛苦减轻，纳差、腹泻等症状减少，同时，西医检验指标也有所改善。总体上看，患者CD4上升、稳定和下降的各占有三分之一，病毒量前三年无明显差异，而到第四年好转情况明显，具有统计学意义。他认为，中医药治艾关键在于辨证求因，审因论治，这已成共识。目前，中医药治疗艾滋病已从以关怀性治疗为主提升到以医疗救治为主。

在项目一线坚持工作了5年的河南中医学院一附院张怀亮教授总结了四个“没想到”：一是没想到中药汤剂治疗艾滋病有效；二是没想到中成药治疗艾滋病有很好的效果；三是没想到中医药对患者免疫缺陷有很好的救治作用；四是没想到中医药救治会受到患者们的广泛欢迎。

确保疗效至上

坚持贯彻“三统一、三结合”原则，保障项目顺利实施和运行质量，推动临床科研同步提高

驻马店市上蔡县李斯楼村是项目的一个医疗点，记者来到这里时正赶上省里专家巡诊。河南中医学院三附院的蒋士卿教授与县中医院的李金山医师一起接待患者。他们依次询问患者近期服用“艾益康”的情况，并记录下来，对出现发热、皮疹或疼痛症状的患者则在诊察后开出处方。记者注意到，每位患者都有项目身份证明本、病例观察本和处方本，巡诊专家必须如实填写并签名负责，相关资料每年底都要汇总到项目办，为进一步开展科研提供基础数据。

诊室内医患亲切交流，面对记者，来应诊的患者并不避讳谈及病情。一位候诊的中年妇女说，她的CD4当初最低降到39，后来服用抗病毒药物和中药治疗，最近刚检查出来的指标是500多。原来她肝脏、胆囊都有问题，浑身没劲，走不动路，还经常腹泻、发烧，“现在很少生病，也有力气干活”，因为孩子上学，丈夫在外工作，她自己在家一个人养着“8大8小3个老”共19头猪。另一位身材敦实的男性患者告诉记者，他一直服用中药，没有用抗病毒药物，CD4稳定在400多。

在牛庄卫生室，几位患者谈到服用中药后的身体情况，首先说的一句话就是“感觉有劲了”。一位30多岁的妇女说，“我刚才还从地里搬运成袋的苞谷哩。”卫生室负责人介绍，目前项目患者40人中，CD4指标上升的占一半，保持平稳的近一半，下降的只有三四人。其中有13位单纯服用“艾益康”的患者，各项检验指标与中西医结合治疗的几乎没有差异。

县级中医院是项目实施的重要依托。上蔡县中医院院长关华告诉记者，医院为每个项目村选派了骨干中医师，在省级专家指导下定期到村巡诊发药，并做好规范记录，对巡诊专家开出的中药处方，医院负责统一煎药装袋送到患者手中，对病情较重的患者，医院开设特设病房，由省、县中医专家会诊治疗，同时还担负着单服中药（小样本）的病毒载量采血体检的具体工作任务。

驻马店市卫生局副局长、市艾防办副主任聂勇经常到上蔡县检查指导工作。在上蔡县中医院艾滋病房区，他对记者表示，中医药防治艾滋病疗效确切，发挥着重要作用，为此，驻马店市成立了相应的领导机构和专家组，大力支持和配合项目开展。

在项目实施过程中，临床科研围绕艾滋病中医证治规律、中医药干预、疗效评价标准等方向，相关研究卓有成效。2006年成立了“河南省中医药治疗艾滋病临床研究中心”，2008年底获得国家“十一五”科技重大专项课题，已经在全国范围内正式启动。中医药对艾滋病机会性感染和HAART毒副作用的研究，被列入国家科技重大专项合作课题。

专家情动中原

5年来，坚持在中医药治艾一线的中医专家们的故事令人动容，其精神令人感佩

李发枝是最早进入中医药治疗艾滋病领域的专家之一。年过花甲的他没有被分派巡诊任务，但是，多年来，他每周二下午几乎雷打不动地到尉氏县中医院艾滋病房参与会诊。那里都是一些危重患者，病情复杂棘手。看完病，往回赶的时候常常已是夜色深沉。在对肺炎、脑炎、痴呆以及顽固性腹泻等艾滋病并发症患者的临床救治中，他积累了丰富的经验。他主持编写了关于中医药治艾辨证要点的培训教材，每年都亲自为项目技术队伍进行培训。

巡诊专家队伍从最初的20人发展到现在的39人，他们大都是科主任、学术带头人，临床科研任务繁重，但每月必到巡诊点出诊两天，“除非是手破了”。他们一次巡诊往返往往达数百甚至上千公里，每人要负责100名左右患者，看病历、开处方、会诊。因为项目经费有限，专家们每月只有80元巡诊补助，但他们年复一年，坚守岗位。

5年前项目启动的时候，许多巡诊点条件简陋。河南中医学院一附院脑病三区主任王宝亮教授负责周口市商水县的3个村125人的巡诊，其中两个村最初只能在村医家里看病，或在村街上摆个桌子。有一位患者病情较重，卧床不起，他就带上医疗器械到患者床边诊治。也有个别患者情绪悲观，不配合，有一次，王宝亮上门看望一位患者，患者死活不肯见，后来又暴躁地拿起铁锹把他赶出家门。蒋士卿也在街上摆过“摊”，用患者家里的桌子，赶上阴雨天，也是患者拿来雨伞。在数年的相处中，专家与患者之间的信任和感情日益深厚。

上蔡县中医院医生李金山是基层中医的代表，按规定他半月下村一次即可，但他主动每周一次。他经常上门看望患者，对每一个患者的情况了如指掌。对重病号，他就在患者家里帮忙熬药，甚至陪伴到半夜。

5年前，呼吸病专家、河南省中医药研究院的徐立然教授，皮肤病专家、河南中医学院一附院的何英教授奉调参加项目办管理工作，从此与中医药防治艾滋病事业结下难解之缘。项目实施之初，他们奔波于试点地区协调布局建点，经常上午跑一个县、下午跑一个县，“天天不着家，一直在路上”。项目展开后，作为项目办的负责人，徐立然把更多精力放到组织专家巡诊、检查督促落实、汇总科研信息等工作中。100多个项目村，他都跑到了，而且不止一遍两遍。作为中医专家，每到一地，他也经常受邀参加重症患者的会诊，与艾滋病患者“零距离”接触。
