艾滋病防治深度观察：让高危人群浮出水面

“防艾是一场战争，打仗就要攻其要害。艾滋病首先在高危人群中传播，能否全力防控病毒在高危人群中流行，决定着这场战争的进程。”
吴尊友，医学博士，中国疾控中心性病艾滋病中心主任。从上世纪90年代起，他就开始近距离接触娱乐场所服务人员。
在云南陇川的一家娱乐场所，吴尊友显然是一名不寻常的“常客”。与一般人不同的是，这位戴眼镜的学者似乎并不需要“服务”，也不特别对哪一名女性感兴趣，而是不停地问东问西。
安全套的使用率，直接影响着艾滋病的传播速度。从高危人群逐渐向一般人群蔓延，这也是全球艾滋病流行、传播的规律。
吴尊友把他10多年学到的国外有关预防艾滋病知识，改装成一册册只有巴掌大的“小人书”，里面是连环画，配着简单的说明。
有一次，吴尊友和助手走进一家娱乐场所，几名女性服务员很高兴，以为来了“客人”。而听吴尊友说明来意后，却都冷淡起来。任凭他怎样解释，就是不理不睬。两个多小时过去了，她们对桌上的连环画初稿仍不屑一顾。中午11点多钟，她们闷头吃起了午饭。这时被晾在一边的吴尊友也不客气，说了声“我也饿了”。然后，他抓起她们的包子就吃，用她们的纸杯倒上水就喝。就这样，一个小小不言的举动一下拉近了他们之间的距离。她们改变了态度，认真听吴尊友讲解，看他设计的连环画草稿，告诉他哪里没看懂，哪里不明白。吴尊友一边听，一边修改，直到她们全部看明白为止。
吴尊友和娱乐场所服务人员成了朋友，并把她们培训成调查员，在自然状态下调查了50名“客人”的安全套使用情况，他们当中近一半的人不使用安全套，而服务者主动提出要求使用的也仅占9%。
2004年，与“禁播”抗争了5年后，安全套公益广告终于登上了央视第一套节目。之后，此类防艾广告频频亮相银幕，国内娱乐场所也开始100%摆放安全套。在此基础上，吴尊友协助国家有关部委制定、完善了安全套推广相关政策，将干预试点项目经验编写为全国技术指南，并承担起负责组织全国安全套推广培训与技术指导的重任。

有人曾在放大镜下看安全套，发现乳胶上有很多小孔，因此质疑安全套的有效性。吴尊友通过文献检索和实验证明，艾滋病病毒不能穿过乳胶的孔。这说明，安全套预防艾滋病性途径传播是有效的。

像阿美一样的女性吸毒者普遍“以淫养毒”。她们通过卖淫获取毒资，将艾滋病病毒从吸毒人群向正常人群扩散。已感染艾滋病病毒吸毒女性的性乱及卖淫行为，成为艾滋病从吸毒人群向正常人群扩散的桥梁。
“回望过去20年，我们对疫情走势有一个逐渐认识的过程，最早的监测思想是御艾滋于国门之外，目光盯在出入境口岸，监测哨点大多设在大城市。”吴尊友说，当疫情突然在西南边境暴发时，地点偏僻，只有146名感染者的吸毒小群体，有人误以为很难有大范围的传播。然而，就是西南边境疫情的星星之火，渐渐蔓延到全国。
吴尊友去西部边疆某地区讲课，当地卫生部门领导向他反映，现在我们这里感染者的人数很多，国家已经宣布了“四免一关怀”政策，我们一个病人都还没有治疗，赶紧帮我们调配一些抗病毒治疗的药物。回京后，吴教授向卫生部有关领导报告情况，加急调运了100人份的抗病毒药物运往当地。出乎意料的是，这些药物在当地却发不出去，由于过去是匿名检测，不知道到底是谁感染了艾滋病。
这件事深深地触动了吴尊友。只有深入那些艾滋病的高危人群中，改变能够导致艾滋病的传染传播行为，才能从源头上阻断艾滋病。找不到感染者的干预，相当于大海捞针、盲人摸象。
吴尊友认为，在吸毒人群艾滋病防治工作中，已感染艾滋病病毒的吸毒者作为传染源，应当最大限度地得到管理和控制，首先要找到他们。吸毒人群的隐蔽性增高，如果找不到他们，就无法开展咨询检测、行为干预等预防治疗服务。
1997年，吴尊友开始在云南的吸毒人群中尝试清洁针具交换。这一行为起初遭到了很多误解，认为那是鼓励吸毒。对此，吴尊友解释为“两害相权取其轻”。
在云南陇川，一位30多岁的吸毒人员用菜刀砍断自己的一根手指，表示戒毒的决心。这位断指明志的吸毒人员伤口还没有愈合，就像其他吸毒者一样，重回到复吸的老路上。
一段时间，吴尊友眼前一直浮现这名用纱布包着手指的男子身影。吸毒成瘾后戒断困难，不是简单的个人“毅力”问题。最新的研究表明，吸毒成瘾后，人脑的一个特定部位发生了器质性病变。这种器质性病变与成瘾有关，目前不可逆转。吴尊友解释说，这一发现改变了人们过去对吸毒的看法，吸毒后复吸实际上是一种疾病。这将促使有关部门重新思考对待吸毒问题的政策和策略。

小强(化名)是一位感染艾滋的Money Boy(依靠性交易赚钱的男同性恋)。在2004年4月19日，作为“零号”的小强(男同性恋中扮演女性角色的一方称为“零号”，扮演男性角色的称为“壹号”)，在北京某洗浴中心与林先生一见钟情，从此开始同居生活。
在北京南三环附近不到10平方米的小屋里，小强捏着一瓶拉米夫定片，和吴尊友漫不经心地聊天。小强已经被检测为HIV阳性，尽管是林先生陪伴着小强度过失魂落魄的一天，但林先生不愿面对现实。吴尊友向小强建议：“带着你的伙伴去检查一下吧。”

2008年3月至2009年6月，一项针对男同性恋群体的调查在全国61个城市悄悄展开。 “大摸底”共分三轮，调查5.3万名男同性恋者。

目前青年学生感染艾滋病的人数不断增加，特别是20岁左右的大学生。重庆某医学院的一位大三男生，前两次检测都是阴性，第三次却呈阳性。吴尊友问他为什么不采取保护措施，这个有医学背景的大学生说：“只要他和我好就行。”

昨天，大雾散去后，冬日难得一见的阳光照在大地上。在荣昌县盘龙镇龙腾社区里，26岁的王石（化名）固执地站在阳光下。这种对于正常人来说是一种享受的动作，对王石来说，却是一种折磨。因患上艾滋病，他的眼睛对于阳光特别敏感。没站多久，王石的眼泪就已经流了下来。他在等，等一些本应与他的命运毫无交集的人来看他。
他从14岁开始吸毒
来看王石的是荣昌检察院检委会委员张思平。“王石从14岁开始吸毒，到2008年时，被检查出感染艾滋病。”张思平说。
到了王石家，王石的父亲和母亲都在。虽然外面太阳很大，但是王石家里很黑。王石的母亲拖出几张板凳，请大家在院子里坐。
“王石眼睛受不了，我们进屋坐吧。”张思平熟悉王石的情况。但是王石固执地要求坐在院子里。
在院子里坐下，张思平坐在王石侧面，帮王石挡住一部分阳光。
“现在情况怎么样了？”张思平把检察院送给王石一家过年用的米和油交给王石后问。
“下周才能去查结果，然后才能领到药。这几天感觉人很累，有时喘不过气来。”王石回答。
“没有再碰那东西了吧？”张思平用词很小心。
“没有了。”王石回答。

在王石眼里，穿着检察制服的检察官已经是自己的亲人。虽然认识这些亲人还不到一个月。“要是我能早点认识他们，我就不会走到今天这一步了。”王石说。
重建被毒品毁掉的家
“第一次我们去王石家时，他没在家，只是清楚这是一个特别困难的家庭。”昨天，张思平回忆今年1月20日，与县检察长梁经顺一起到王石家的情况。
当时，就王石的父母在家。通过王石的母亲，梁经顺知道，这个家庭有两个儿子。小儿子正在广东读大学，而大儿子则已经离家出走，而且身染艾滋病。
这是一个被毒品毁掉的家。家徒四壁，小儿子大学的学费，也是亲戚朋友们借的。
“当时，梁经顺把身上带的1000元钱，全给了这个正无法过年的家庭，并当即决定，让荣昌检察院和这个家庭结成穷亲。”张思平说。
之后，荣昌检察院数次到王石家里，帮助这个家庭的生活。并负担起王石弟弟的学费。到了7月，王石的弟弟回到重庆，在当地检察院实习。在这期间，检察院完整地了解到了王石的情况。
原来王石14岁时出门打工，结果遇到坏人，被拖下了吸毒的深渊。为此，他的父亲已经气得神志有一点不清楚了。到了2008年，王石在成都被查出感染艾滋病。得知病情后，王石自杀未果，被送回了家。
而现在，王石因为愧对家庭，已经离家出走了。王石的弟弟学医，知道如果哥哥的病情得不到控制，很快就会死亡。因此请求检察院，一定要帮忙找回哥哥。而这时，大家仅知道一个汇款地址。此前，王石在山东打工存了几千元，因为担心忍不住拿去吸毒，他曾把钱汇给弟弟。后来实在经不起毒品诱惑，又打电话从弟弟手中要回那几千元。家人由此留下一个地址和电话。
检察官劝回离家浪子
张思平说，当时大家都只知道王石在山东，但是在山东哪里并不清楚。而且王石曾打电话回过家，但那是一个公用电话。
荣昌县检察院与山东检察系统进行了联系。通过他们，找到了王石。每一次电话打过去，王石很抵触，不愿回家。
随后，荣昌检察院多次电话联系王石，并通过王石的弟弟做王石的思想工作。终于在今年12月1日，王石回到了家里。第二天，检察官就赶到王石家里，与他建立了联系。荣昌检察院将他纳入了社区矫正体系。不时有检察官去王石家，让他重燃生活的希望。
从现在来看，王石已经走了出来。王石说，感谢检察官让他懂了法，懂了快乐的意义。
“我们会一直关注王石的情况，帮助他走完人生剩余的旅途。而且，王石家的困难，我们也会一帮到底。”张思平说。
