防艾，基层还有哪些空白要补

广西壮族自治区艾滋病流行形势十分严峻，感染人数逐年增多，给人民群众身体健康和经济社会发展带来了严重危害。最近，广西疾病预防控制中心在14个市109个县(市、区)进行了艾滋病防治调研，以厘清艾滋病防治面临的问题。
感染“家底”怎样才算摸清
作为流行病学专家，广西疾控中心主任唐振柱认为流行病学调查是艾滋病防控最关键的第一步。广西在艾滋病监测检测方面已经达到了县级医院全覆盖，部分乡镇卫生院也开展艾滋病初筛工作，但是调研组在某县调查时发现，县里汇报的艾滋病病毒感染者和病人的地区分布只统计到乡镇一级。唐振柱认为，这不利于找出艾滋病的重点村屯或社区。因此他要求，各级防艾专业人员一定要把疫情分布统计到具体的村屯或社区一级。
比如有一个县，1999年发现首例本地艾滋病病毒感染病例后，艾滋病病毒感染者逐年增加，截至2010年10月底，累计报告艾滋病病毒感染病例135例。而细化到全县10余个乡镇，高发的只有3个乡镇，依次为53例、31例、24例，占了全县的大多数。这3个乡镇之所以高发又主要集中在数个村屯，是与其吸毒人员数量、人口流动情况、社会经济发展情况、暗娼的活动等因素有关。
“只有这样，我们才算摸清了艾滋病感染的家底，才能开展有针对性的防治行动。”唐振柱说。
“保密”不该成为防艾屏障
在某县妇幼保健院初筛实验室里，调研组专家敏锐地发现原始记录与网络直报数字不相符的问题。该院2010年1月~10月筛查了3000多名孕产妇，发现3名艾滋病病毒抗体阳性者。在原始记录册上是清楚记录的，可是在直报表上的阳性记录却是“0”。这是为什么？
有关人员解释说：“因为涉及保密问题，我们只是向县疾控中心报告，他们没有反馈结果给我们，不能确诊，我们只好网络直报‘0’了。我们也曾劝3名艾滋病病毒抗体阳性者去市级妇幼保健院开展检查和实施母婴阻断，有2个人去了，还有1个没有去。这个没去的孕妇是农村人，我们打了多次电话，但也只能从健康教育的角度去劝她，没有办法要求她必须去接受母婴阻断。”
调研组当即要求当地疾控中心会同妇保院想办法把这名农村孕妇找回来，帮助她做好母婴阻断的治疗。
调研组在另外一个县也发现类似的问题。某县医院筛查的原始记录今年1月~10月艾滋病病毒抗体阳性者有31名，而网络直报只有21名。还有10名阳性者哪里去了？县医院也说不清楚，只是说因为保密原因，县疾控中心没有反馈，他们也不好追问。直到调研组到市里，才发现这些初筛阳性者身份证等资料不全，所以没有做确认诊断……
阳朔县疾控中心的一位艾防人员还反映，病人随访工作做得不太好，主要是因为忙不过来。本来这件事是可以交给当地卫生院做的，可是随访权没有下放到乡镇卫生院，所以他们未能参与进来。
调研组提出，卫生系统内部要共同参与、配合，要科学认识保密的目的，打破卫生系统内部的“保密”桎梏，形成合力。
特殊群体防艾路径仍须拓展
经过现场开展流行病学调查分析、专项调查等方面技术指导，工作组发现，由于艾滋病预防知识还比较缺乏，部分群众尤其是农村中老年居民自我保护意识淡薄，因此，加强对娱乐场所暗娼的各项干预活动、加大对乡镇低档场所暗娼的监测与干预，已成为艾滋病防治攻坚战的一个重要课题。
在某县疾控中心外展工作人员的帮助下，调研组召开了该县部分娱乐场所业主座谈会。业主和外展人员介绍，这个县的卖淫嫖娼现象十分严重。其中，中低档场所人群流动性很大，一般1个月到3个月就走人了，“打一枪换一个地方”，再也找不到人。
外展人员说，通过长期的干预工作，现在中档场所的小姐防范意识比较强，嫖客没戴安全套她们宁愿不做“生意”，无论多高的嫖资也不做。“难就难在低档场所的干预工作，尤其是站桩、出租屋及小旅社的暗娼，她们流动性大，对工作人员怀有很大的防备心，难以接触。”
还有比这些低档场所更难干预的对象，就是走村串户的流动暗娼小团伙。该县疾控中心估计，约40%的村有这种流动卖淫现象。这些流动暗娼没有任何防护措施，更容易造成艾滋病病毒传播。
调研组希望外展人员多关心这一特殊人群，帮助她们提高自身保护意识，尽最大的努力做好这些人的登记工作，了解清楚她们来自什么地方，告诉她们可以接受免费咨询体检，一旦检出阳性，要立即开展阳性告知，决不能再传播病毒了。

"Confidential" should not become a barrier AIDS prevention
In a county MCH screening laboratory, the research team of experts quickly discovered the original records and the figures do not match the network report the problem.
