每天早晨8点，北京协和医院感染科病房的护士长张艳彬带领护士们开始例行查房。
在护士提醒患者称体重或做询问的时候，有些健谈的病人和她们聊起了自己的往事，偶尔还会和护士开个玩笑。被善意调侃了的年轻女护士自己也笑了起来，对同事指那个病人说：“他就是这么可爱，太可爱了。”
转完一圈，护士们回到一个装着密码锁的房间，分组配药，当配药单做完标记后，大家都戴起了无菌橡胶手套，再次出发去病房，给部分患者打吊瓶。
“刚才的病房与普通病房有什么不一样吗？” 护士赵霞告诉《望东方周刊》，那个与护士开玩笑的“可爱的”病人，是一个艾滋病毒携带者。
作为收治艾滋病人和病毒携带者的病房，这里并没有任何人们想象中的焦虑和不安。
“我们也都经历过胆怯和紧张，刚来感染科的时候，我们连给患者输液都不敢，都是主治医生亲自动手，有这样的榜样在那里，我们也很快适应了，”赵霞说，“艾滋病患者在护理过程中，最怕的就是有创治疗，比如输液打针，但只要措施得当，操作规范，用我们主任的话说那就是‘想被传染都是很难的’。”
护士长张艳彬说，“来到我们科就诊的都是已确诊的（艾滋病人或病毒携带者），我可以很骄傲地说，我们科的护士从来没有一个人说轮到她输液了，她会说不的。不仅是医疗护理，护士们日常的工作还包括给患者洗头、洗脚、剪指甲。我们平时会告诉大家思想不能放松警惕，但是，这不是只有在护理艾滋病人时才需要的，是在每个科室任何时候都需要的。”
“在这里，留下的护士都是最棒的。” 赵霞对自己的工作很自豪。
最危险的地方也是最安全的地方
每周二、三的上午是协和医院感染科专科门诊时间，七点半左右，护士宋晓璟拉着一个黑色皮箱准备赶去门诊部，看上去像是个“白衣”空姐。
“你怕吗？”宋晓璟对本刊记者的发问提示了这个门诊的特殊性。
这是李太生教授的专家门诊，2000年他在协和医院创办了HIV实验室，2001年开始接受第一批患者，患者资料都被严格记录用来做药物治疗分析。在这个实验室里工作的护士被称为“专科护士”。
门诊部里一上午都很忙，李雁凌指着早晨拖来的皮箱说，“这里装着所有今天要来复诊的HIV感染者的病历档案，详细记录着他们患病情况和用药情况。现在医疗条件好了，尤其是2008年国家对HIV部分药品进行免费治疗之后，病人数量多起来了，我们的工作也越来越复杂。”
在这里，护士和病人的关系很特殊，对话的主题除了病况，还有不少生活的话题，好像熟人、朋友一样。有个患者蹭到护士耳旁说：“我昨天喝酒了，吃的药全吐了，怎么办？”表情就像犯了错的孩子。
李雁凌在这里已经工作了6年，她说：“当时医院选拔‘专科护士’，而专科护士在国内刚刚兴起，还需要通过考核，有一定挑战性，我就主动想试一下。我丈夫也是一个医务工作者，当初选择来艾滋病科的时候，他说，‘去吧，最危险的地方也是最安全的地方’。”
转个圈便能看到阳光
李雁凌至今还记得她刚来到专科病房工作时见到的一个病人。“一天中午，医院通知我在妇产科就诊的一个女孩被查出是HIV感染者。我过去的时候，看到她正在独自落泪。我花了两个多小时和她谈话，她也渐渐答应会接受治疗。可是，3个月之后，依然没有看到女孩前来就诊。”后来李雁凌主动给那个女孩打了电话，但她几乎没有回应就挂断了电话，此后就再也联系不上了。
“我希望她在别的地方接受了治疗，如果她放弃了，那是我最不愿意看到的。”李雁凌说。从那个女孩的消失起，对于这些被社会当成异类的“专科病人”，她就多了一份特殊的惦记。
在李雁凌的患者中有一位70多岁的老人，也是在治疗其他病症时查出来的。当时老人坐在轮椅上，眼睛里充满绝望，嘴里喃喃说：“我怎么得了这么一个脏病？”无论护士怎么劝说，他都不接受治疗。就在李雁凌推着他进入抽血室的时候，老人突然用双手撑住门框，声嘶力竭地大喊。
这个老人的放弃对于李雁凌来说是又一次打击。“我们每次都尽量在第一时间赶去疏导病人，可是我们没有办法强迫病人接受治疗。” 李雁凌她们意识到，这类病人最煎熬的阶段是在刚知道诊断结果之后，那个时候的心理调试很重要。“于是我们就想方设法，让他们在第一时间明白，感染了艾滋病毒，就像得了糖尿病和高血压一样，只要终身服药，他们依然可以过得和平常人一样精彩。”
“有的时候，患者当时想通了，过一阵又想不通了，反反复复，我们的思想工作也就持续地做。”李雁凌说。
有个患者一度治疗得不错，某次来就诊的时候忽然出现了肺炎症状，李雁凌便问他：“前两天不是还挺好的吗？你按时吃药了吗？”他灰心丧气地说，“昨天又想不开，去淋雨了。”这个患者的情绪波动特别大，常常来实验室和护士絮叨着一些不开心的事情。
“我就告诉他，这儿很多患者在知道病情后会更加珍惜生命，努力规划人生，不少人现在工作生活上都做得很出色。后来，这个患者也找到了一份工作，开朗了很多。”
“我们常常容易会被患者的悲伤故事所打动，被带进他们的灰色心境，可是我们又必须及时把他们从那个封闭的圈子里拉出来，他们有时只是背对着阳光，我们需要做的就是给他一个动力，让他转个圈，他们马上就知道阳光在哪里，勇敢地面对，而不是逃避。”李雁凌说。
也被他们感动
在HIV实验室里，从医生到护士手机都24小时开着。患者都有他们的联系方式，不论是休息日还是晚上，患者只要来电话了，他们就会帮助解决问题。
艾滋病患者的用药很复杂，多达十几种药，必须按照一定比例顺序服用。护士每次会和患者交代两遍，再让患者复述一遍，好像老师指导学生背书。
但复杂的问题还是会出现，比如病人吃了药后呕吐，很难弄清楚哪些药已经吸收了，哪些药要重新服用。尤其是，这些患者通常都不愿被别人知道自己在吃药，在公共环境里很可能因为紧张吃错了药。
“我们科的患者和其他病人有个巨大的区别，那就是他们往往没地方倾诉，没家属支持，护士这时候就必须承担一些护理工作以外的功能，比如听他们倾诉，给他们安慰。”李雁凌说，“但他们也常常让我们感动了”。

一个患者每天要上班，还照顾着朋友的母亲，生活辛苦，但他特别开朗。“一次他在我们这儿聊了一下午，在他走后，我和雁凌都哭了。”宋晓璟说，“没想到，这个患者担心自己影响了我们的心情，还给我们传来Lady Gaga的音乐，逗我们开心。他们特别知道感恩，特别细心，特别坚强，他们常自嘲的是‘我不坚强还能怎么办’。”

宋晓璟告诉本刊记者一个故事：在超市结账的时候，突然发现自己身后一个人也没有，而别的收银台排满了人，“原来我手里拿着几份艾滋病患者的材料，封面印着大大的‘艾滋病’字样”。
类似的事经常发生。当宋晓璟在业余英语口语班上做完自我介绍后，听见下面一阵议论纷纷，老师也用惊讶的口气反问，“艾滋病？！”
“烦并快乐着”，对工作意义的认同感让这些艾滋病专科护士“淡定”地面对职业带给生活的种种困扰。
