“阴滋病”疑云：最好的辟谣是全体治愈

　　除了消瘦，杨诚看起来并无异样。“我只是看起来正常，其实身体有很多症状。”他露出一丝苦笑。因为最近出现的持续低热，说话间他的鼻子不断沁出密密的汗珠。

　　5月9日晚，广州，本报记者见到了这个26岁的小伙子，杨诚是这个群体的活跃核心成员，在钟南山征集的60名病患志愿者中，他第一时间联系钟南山的助手，第一个住院检测。

　　事情开始于2010年7月9日的一次高危性行为。在那之后，这个群体共有的症状绝大部分在他身上一一出现，关节的疼痛和弹响则由始至终。“很多症状不是持续的，断断续续，这个好点那个就严重些。”

　　和这个群体中几乎所有人一样，一开始杨诚怀疑自己得了艾滋病。在满了六周的窗口期后，他到广州市第八人民医院，检测出艾滋病毒阴性。

　　满两个月、三个多月的时候，杨诚分别又做了一次HIV检测，还是阴性。三次以后，他再没测过HIV。“我这个人比较干脆利落。一开始我是恐艾，但三次检测全阴我就脱恐了，更多时间是花在找病因上。”

　　他很快在网上发现了这个群体———“阴性艾滋病人”，也有人说是“未知病毒感染者”。随着时间推移，他发现自己与他们的共性越来越多，他不得不承认自己就是其中一员。

　　“专家对我们这个群体有一种错误的印象，因为这个群体混在大量恐艾症的人群中。恐艾症是典型的强迫症，见到医生就抓着聊天，问感染的几率有多少。我不恐艾，我能吃能睡，恐什么？”

　　杨诚还举了一个例子，山东的老鞠，传染了女儿，“四岁半的小孩关节也响，她恐什么？”

　　杨诚还发现自己传染了同事。三个同事出现明显症状，“关节响，一坐下一站起就能听到，还有不明原因的咳嗽。”有个同事乏力嗜睡，上班途中昏倒在路上，摔得满脸是伤。他猜测是聚餐时传染上的。

　　他辞去了原先的工作，建立了专为“寻找真相的人们”沟通交流的论坛“生命之声”。这个橙黄主题的论坛宗旨明确：网聚病友的力量，科学收集病情，向世界传播我们的声音。

　　“我们得自救。”杨诚相信，他们这群人是被某种未知的病毒感染了，不然为何各省的病友都没有查出病因？

　　而绝大多数专家并不认可他们的观点，或者表示无能为力。

　　四川病患“圣经哥”在电话里告诉记者，他光检测费花了好几万，专家怀疑的和自己怀疑的项目都做了，无果。感染了三年，传染了家人，怪病依然得不到治疗。

　　在记者加入的QQ群里，每天都有人在讨论病情，对比症状异同，给出吃药的建议，然而多半是针对具体症状的，比如服用什么牌子的抗生素。也有人推荐中医疗法，但在这个没有专业知识的群体里，更多人是茫然地摸索。

    “专家认为这不是一个公共卫生问题”

　　2011年对于杨诚他们来说，实在是波澜起伏。

　　从2009年起，这个群体就不断在呼吁求告，希望被认可存在，得到“正名”，还陆续有病友找到卫生部信访办反映情况。

　　今年4月初，卫生部正式作出了回应。卫生部新闻发言人邓海华在4月5日和4月11日反复辟谣，称该人群没有艾滋病，只是“恐艾症”的表现，不存在阴性艾滋病，人群所患的也不是某种传染病，相互之间不会传染，因此公众对此无需担心。“专家认为这不是一个公共卫生问题。”邓海华说。

　　卫生部的说法主要基于两次调查：一次是中国疾病预防控制中心(CDC)流行病学首席科学家曾光2010年1月对59名自愿报名的病患所作的检查；另一次则是杨诚今年2月底上访后，卫生部对六个省市开展的流行病学的调查。

　　曾光先后给这个群体写过五封信。2009年7月18日，第一封信中，曾光表示：“你们的诉求已经成为了需要认真调查解决的公共卫生问题。”

　　2010年1月10日至17日，由曾光助手裴迎新联系挑选的59名患者在北京地坛医院接受了检查，59名患者包括54男5女，平均年龄在33.6岁，检查项目包括常规体检(内科、外科、肿瘤科、五官科和皮肤性病科)、实验室检查肝肾功能、血糖和血常规，CDC性艾中心进行性病艾滋病特异性检查。

　　当年2月11日，地坛医院出具体检报告。“总体分析，该组人群多数无明显器质性病变，少数生化检查异常者，亦与其主诉的临床症状不相符合。从躯体症状和神经症性症状两个角度进行评估，考虑主要为精神因素所致。”

　　这次曾光主持的调查显然没能让病患满意。

　　杨诚告诉记者，今年2月底，20多名病友再一次联合到北京上访。

　　这一次，杨诚等人见到了“大人物”———卫生部疾控局副局长郝阳、中国疾病预防控制中心性病艾滋病防控中心主任吴尊友。“他们说马上要在全国范围内针对此事开展流行病学调查。”

　　然而，起初的“很高兴很期待”又化为失望。病患们发现，这个调查范围非常窄，选取的北京、上海、江苏、浙江、湖南和广东六省市，每个地方名额都很有限，最后调查人数不过40多人。

　　“调查针对性不强。我们普遍反映的关节有问题、头痛、肠鸣和淋巴结肿大等，都没有做对应的检测。而且6个省市检测的项目也不一样。”杨诚认为这违背了流行病学的调查原则，“应该查找共性。”

　　作为广东省被调查对象之一，杨诚最后只收到自己的检测结果。这次流行病学最后也没有对外(甚至是对受检群体)发布一个结论。

　　回忆起一个月前，杨诚说：“我们基本上陷入了绝望，对国家相关部门不再抱任何希望。媒体铺天盖地地说我们是‘恐艾’、‘精神因素致病’，卫生部这么权威的部门说我们没有病，我们还能找谁？”

    “要不是钟南山把‘恐艾症’的帽子摘掉，我们永远没有出头之日”

　　就在几乎山穷水尽的时候，5月6日，钟南山率领广州医学院第一附属医院的团队召开专家研讨会，并于会后向媒体公布了他们从3月31日至5月3日历时一个多月的调查结果。

　　钟南山表示，今年他去参加全国两会前，就接到很多病人的短信和电话，也有媒体希望他能对这些病人做个科学分析。于是钟南山组建多学科团队，对自愿报名的来自16省市的60人进行检查。其中病人50名，42男8女，还有10名家属，平均年龄34.2岁。

　　最后得出的结论是：排除感染艾滋病可能性，不完全由精神因素致病，48位病人查出不同程度的疾病，即80%感染一种或多种病原体，但皆为常见性传染病原体，其中33人感染EB病毒，这些病原体中绝大多数可通过性传播途径传播。精神因素加剧了病情，自愿接受心理检测的12人中有7人心理异常。

　　“检测的项目非常多，以前从没检测这么多过。”杨诚说。

　　最终，杨诚只检测出三项异常：双膝关节轻度退行性变；在前列腺液标本中发现淋球菌；白细胞异常，免疫力差，补体C3低。

　　他有些想不通：“淋球菌感染就是淋病吧，我的生殖器各方面没有症状，我的症状好像跟这个检测结果不大符合。”

　　但他这一次很平静，“对目前的结论持保留意见，先相信钟院士的结论，治疗已知病症，再观察症状。”

　　“我相信以钟院士的责任感，如果他发现我们的病症不完全由这六种病原体造成，会继续帮助我们走下一步，我们对他很有信心。”杨诚说。

　　“不管怎样，我们对钟院士是非常感激的。在全国的医院、专家还有行政部门都说我们是精神因素致病的时候，钟院士扛着压力介入研究，这个功绩是非常大的。”

　　病人们对于这一点深怀感激，“要不是他把恐艾症的帽子摘掉，我们永远没有出头之日。他用科学给我们平反。”

    全国患者都找钟南山也不是个办法

　　5月10日，在卫生部例会上，卫生部新闻发言人邓海华对钟南山团队发布的结论作出回应：钟南山的研究成果与卫生部的调查结果总体上是一致的。没有证据表明该人群所述疾病具有传染性和聚

　　集性，没有临床、实验室和流行病学证据支持该人群患有某种传染性疾病。

　　这与4月7日卫生部发布在官网上的辟谣调查结论是同一句话。

　　至于EB病毒是不是这个人群致病的原因，邓海华说，他们咨询了EB病毒研究专家曾毅，曾毅认为EB病毒是健康人群中很常见的一种病毒，三到五岁儿童的感染率达到90%以上，而且以后是终身携带，一般不会引起这个人群所说的这些病症。

　　当天下午，钟南山向媒体表示，“我不完全同意卫生部的说法。”他称，美国已有研究机构证实了EB病毒的传染性，他正在通过海外的朋友查找相关的资料，要有依据才能做出回应。他会在收集资料后，尽快做出明确回应。

　　对于公众关心的后续研究问题，5月10日下午，记者先后采访了钟南山团队中的两名医师，分别是感染病科主任曾文铤和负责病原体检测的呼吸疾病国家重点实验室周荣博士。

　　曾文铤表示，自5月6日的专家会之后，团队未再做沟通协调，他作为负责临床的医生，会做好全国患者来诊的接待工作，做好常规检查治疗。研究方向上，“我们的资料要继续完善，会根据研讨会上专家提出的问题完善检查。”

　　周荣在接受记者采访时，委婉表达了对于卫生部关于EB病毒不具传染性这一表态的不认可。他坦承，排查未知病毒的难度很大，国内目前有三家机构在做未知病毒的排查，但并非针对这个群体。“我们的结果只是初步的，提供给专业人员参考，还有待专家的认可。”周荣说。

　　杨诚也表示，病患们都希望，钟院士的结论只是刚刚开始而不是结束，过段时间能有一个病情治疗的中期发布会，“还远没有到结束的时候。”

　　杨诚说他看到一则时评，有一句话让他很触动：“最好的辟谣就是把这个群体给治愈。”全国患者都来找钟院士的团队也不是个办法，如果能在全国各地开展专家研究和医疗机构的对症治疗，对这一人群的病情控制绝对有好处。

　　“无论怎样，这个皮球最后还是要踢回卫生部。”杨诚认为，卫生部应该抱着科学态度，“不要一下子说死了，那就万马齐喑了。”（文/片 记者 廖雯颖）
