河南中医药治疗艾滋病取得阶段性成果

中广网北京11月26日消息（记者刘天思）艾滋病，中医学对艾滋病尚无统一认识，有“温疫”说、“伏气温病”说、“虚劳”说等。由于中医学对各种疾病的认识是通过疾病的临床表现，以及时令气候、地域环境、生活及饮食习惯、患者体质等因素，加以分析、归纳，推演出各种疾病的病因病机，然后根据其病因病机确立治疗法则，选择符合治疗法则的恰当方药，予以治疗。即“辨证求因”、“审因论治”，也就是“辨证论治”。因此，对艾滋病病因病机的探讨，对该病的治疗有重要指导意义。
传统意义上的“疫毒”，在性质上有风、寒、热、燥、湿等不同，但最多见的则是温热之邪。而艾滋病“疫毒”侵袭人体以后，虽然在急性感染期可出现短暂的温热或湿热邪气的一些特性，但其后的无症状期及艾滋病期，则主要表现为损伤人体正气的特性。在河南所观察的1792例HIV/AIDS病例分析（均为无症状期及艾滋病期），多数患者首先表现为脾气亏虚（如消瘦、乏力、泄泻、汗出、易感冒等），进而导致五脏气血阴阳俱虚。并在疾病过程中伴有或风寒火热，或痰饮水湿，或气滞血瘀等邪，这是疾病的根本机制。
在掌握了疾病本质之后，河南制定了治疗法则为，健脾益气，滋阴养血，祛风清热，化湿解毒，即扶正为主兼祛邪。在张仲景《伤寒杂病论·金匮要略》选用薯蓣丸为基础，进行加减化裁，制成益艾康胶囊，进行临床观察。
通过对河南省1792例HIV/AIDS患者（均为有偿献血感染者）五年的临床治疗观察，取得了阶段性成果。
观察方法
HIV/AIDS临床诊断标准按照卫生部、中华医学会《艾滋病诊疗指南》标准执行。
HIV/AIDS中医辨证标准依据国家中医药管理局颁布《中医药治疗艾滋病项目临床技术方案》，《河南省中医药治疗艾滋病常见病症辨证治疗要点》。
制订了纳入标准和排除标准。
病例共分为两组，即中药组和中药加抗病毒组。
中药组 益艾康胶囊（人参、黄芪、炒白术、茯苓、当归、川芎、白芍、黄芩等组成）每日3次，每次5粒，温开水送服。中医辨证施治根据病人的症候、舌苔、脉象等情况进行中医辨证施治(参见国家中医药管理局颁布《中医药治疗艾滋病项目临床技术方案》及《河南省中医药治疗艾滋病常见病症辨证治疗要点》)。应用煎药机加工为每付中草药250ml×2袋装，每次1袋，每日2次，保质期7天。
中药加抗病毒组选择中成药（制剂）与辨证施治相结合的治疗方法同时，给予抗病毒治疗（HAART疗法）。
疗效评价标准，依据国家中医药管理局《中医药治疗艾滋病试点项目临床技术方案》执行。
治疗结果
目前临床症状与体征显著改善，生活质量明显提高，机会性感染显著减少，病死率逐年下降，取得了较好临床疗效。虽然我们当初的治疗思路，既不是立足于抗病毒，也不是立足于提高患者免疫功能，但阶段性结果显示：病毒载量（119例）下降加稳定者占84.8%，提示中医药有降低或稳定病毒载量的作用，且与中药加抗病毒组比较无统计学差异（P>0.05）；对于CD4+T细胞作用，中药组治疗前后比较，有统计学差异（P<0.05），治疗后有所下降，但与中药加抗病毒组比较，无统计学差异（P>0.05），说明中医药有稳定或减缓CD4+T细胞下降趋势作用。
典型病例
1．张××，女，1969年9月，尉氏屈楼村一组，04年CD4417，随即加入中医药治疗。消瘦，反复发热，失眠，体质极差，长期卧床。经过治疗一年后，可以下床活动，干家务活。09年CD4298，目前，身体很好，从事劳动。现在家养猪，年收入2-3万元。（尉氏中医院）
2．朱××，男，1970年4月，尉氏枣朱村三组，家中4口人。04年CD4608，间断反复发热，胸闷气短，乏力，腹泻并渐渐加重，体重明显下降，逐渐卧床不起。经中医药治疗后，感冒发热1次/年，腹泻已愈，逐渐体力恢复，体重恢复，参加建筑队务工。（尉氏中医院）
3．韩××，男，1973年4月，尉氏屈楼村三组，04年CD4406，乏力，反复感冒发热，体重下降显著，经常汗出。经过中医药治疗后身体逐渐好转，09年CD4443，现在身体状况很好，在家经商，已经致富。
